Reliefsinus Clinical Services

Recurrent sinus infections may be associated with persistent microbial communities, structural factors, or underlying inflammation. Our pharmacists interpret diagnostic findings to help identify contributing factors and guide individualized, targeted treatment strategies to support sinus health.

What is the Reliefsinus Clinical Service?

The Reliefsinus Clinical Service is a targeted approach to managing sinus infections and chronic rhinosinusitis complicated by biofilm. Biofilm is a protective matrix that shields bacteria and fungi from standard antibiotic therapy and the immune system, making recurrent or persistent sinus infections particularly difficult to resolve with conventional treatment alone.

Using molecular diagnostics, our pharmacists identify the specific organisms present in the sinus environment and provide clinical interpretation to guide treatment. This allows for a precise, personalized compounded prescription delivered topically to clear the infection and support long-term sinus health.

How it works

Our PharmD i3 process (Identify, Interpret, Intervene) guides every clinical service engagement:

  • Identify: Molecular diagnostics pinpoint the specific bacteria, fungi, and resistance genes present in the sinus environment.
  • Interpret: Our pharmacists provide clinical interpretation of diagnostic results to support your provider in selecting the most appropriate therapy.
  • Intervene: A personalized TargetedRx is compounded in a topical formulation designed to penetrate biofilm and treat the identified infection while supporting sinus microenvironment restoration.

Who is this service for?

This service is designed for patients and providers managing:

  • Chronic or recurrent rhinosinusitis that has not resolved with standard oral antibiotic therapy
  • Sinus infections with suspected or confirmed biofilm involvement
  • Post-surgical sinus patients experiencing persistent symptoms or reinfection
  • Patients seeking a pharmacist-guided, topical approach to sinus infection management

What is Rhinosinusitis?

Rhinosinusitis is a common condition that affects about 1 in 8 adults in the United States and is one of the most frequently reported complaints in family medicine. It involves inflammation of the nasal passages and sinuses and is often associated with infection.

Chronic rhinosinusitis (symptoms lasting more than 12 weeks) may affect up to 12.5% of the population. It has been associated with air pollution, cigarette smoking, allergies, and environmental exposures including carbon monoxide, nitrous dioxide, sulfur dioxide, and particulate matter. Patients with allergies are significantly more likely to experience the chronic form.

Common symptoms

  • Congestion
  • Difficulty breathing
  • Facial pain
  • Headaches
  • Sleep issues
  • Loss of smell
  • Nasal discharge

Why topical therapy?

Medications used for treating rhinosinusitis may include analgesics, antibiotics, antifungals, steroids, and more. Oral antibiotics like amoxicillin are frequently prescribed as a first-line treatment. One significant benefit of a topically administered medication is that it has fewer systemic effects than an oral medication, which is an important consideration for patients already managing other conditions or taking multiple medications.

There is significant evidence that topical antibiotic treatment can help patients with rhinosinusitis, especially those who have already had sinus surgery. Many healthcare practitioners also prescribe topical antifungals based on their clinical experience, even where broad national consensus is still developing.

Topical sinus therapy with a saline rinse

Rinsing the sinuses with a saline solution has been shown to be an effective way to clear the sinuses and provide relief. The best delivery methods include the IntraNase applicator or a neti pot system like the MAOEVER sinus rinse bottle, which delivers a controlled flow of solution into the nasal passages.

Patients seeking sinus relief may find a saline solution effective on its own, but others may need a solution that includes a compounded medication. Nasal lavage can benefit symptoms ranging from minor irritation to chronic rhinosinusitis, even in cases where no signs of infection are present.

Up to80%

Biofilms are detected in up to 80% of patients with chronic rhinosinusitis, forming on the sinus mucosa and protecting polymicrobial communities from standard antibiotic irrigation. Their presence is strongly associated with treatment failure, persistent symptoms, and higher rates of surgical revision.

More than2X

Patients with biofilm-positive chronic rhinosinusitis are more than twice as likely to experience persistent or recurrent disease after standard medical and surgical treatment compared to biofilm-negative patients, highlighting the critical need for biofilm-targeted compounded therapy as part of a comprehensive sinus management plan.

96%

Fungal organisms were identified in 96% of patients with chronic rhinosinusitis in a landmark Mayo Clinic study, with an abnormal immune response to the fungi, rather than direct fungal invasion, driving the persistent sinus inflammation. Patients exposed to water-damaged buildings and mold are particularly susceptible, as ongoing fungal presence in the sinus cavity sustains the inflammatory cycle and prevents resolution with standard antibiotic therapy alone.
